Mercy Health in Ohio has partnered with Cityblock Health to extend care services for Medicaid patients in Ohio.  

The collaboration will serve Medicaid patients attributed to Mercy Health primary care providers in the greater Cincinnati and Toledo areas.

Cityblock will work with Mercy Health’s primary care physicians on primary and urgent care coordination and will extend services for behavioral health and social care. These services will be available to Medicaid beneficiaries 24 hours a day through in-person and virtual care.

WHY THIS MATTERS

The partnership aims to improve outcomes for Ohio’s underserved Medicaid populations and lower costs. The integrated approach will engage hard-to-reach patients, address social needs such as food and housing, and help reduce emergency room visits and hospitalizations. 

Mercy Health is part of Bon Secours Mercy Health, one of the largest nonprofit Catholic health systems in the United States. Mercy Health is among the largest health systems in Ohio.

Cityblock is a value-based provider for the Medicaid population. Its care model targets the challenges facing the Medicaid population, many of whom are at disproportionately higher risk for poor health outcomes, the company said.

All medical services are provided by licensed physicians and healthcare practitioners employed by Cityblock Medical Practice, P.A. 

Cityblock has been operating in Ohio since 2021 and currently serves more than 30,000 members in the state, including at its two community clinics in Columbus and Cincinnati. This new collaboration expands Cityblock’s offerings in Northwest Ohio.
