Elevated Community Health, formerly known as Summit Community Care Clinic, continues its mission to expand access to care across Colorado’s high country through a new membership-based program designed for the local workforce.
Founded more than 30 years ago, Elevated Community Health is a nonprofit federally qualified health center serving mountain communities throughout the region. Elevated provides care regardless of a patient’s ability to pay, distinguishing it from many other medical providers in the area.
Pediatrics to Seniors — Elevated is for Everyone, providing primary care, dental and behavioral health services at every stage of life. Access remains central to its mission, Elevated serves patients who are insured, underinsured or uninsured through locations in Summit, Lake and Park counties.
That commitment led to the creation of Elevated Essentials, a health care membership program designed to address the realities of living and working in mountain communities, where high living costs, seasonal employment and limited benefits often make care difficult to obtain.
Elevated Essentials offers individual, family and small business plans that provide access to core services at any Elevated location. Each plan includes a defined number of visits, with additional visits available for as little as $70 per visit.
Members pay a flat monthly rate for each service, creating a predictable and transparent alternative to traditional insurance-based care.
