SAN ANTONIO – Teens across Bexar County are being asked to weigh in on mental health needs through an anonymous survey that city leaders say helps guide resources and support for young people.
The City of San Antonio Department of Human Services’ San Antonio Youth Commission and Project Worth Teen Ambassadors have opened the fourth Teen Mental Health Survey on the SASpeakUp community portal.
Youth ages 12 to 19 are encouraged to participate. The survey is open now through April 30 and is available in English, Spanish, and other languages.

“The importance of the San Antonio Teen Mental Health Survey is to see the mental health challenges that today’s youth currently face that drastically change their day-to-day lives in how they operate in schools and in our community,” said Tabitha De La Rosa, a San Antonio Youth Commission member representing Council District 3.
City officials said the survey effort began in 2021, when teens created the first survey with guidance from mental health and data experts to better understand how the COVID-19 pandemic affected youth mental health in San Antonio.
The city said results from that work helped position San Antonio to allocate $18 million in one-time pandemic relief funds to support teen mental health programs in 2022 and 2023.
Insights from the 2024 survey found that many teens turn first to friends and peers when struggling with mental health challenges, including substance abuse, according to the city.
In response, Project Worth Teen Ambassadors and the San Antonio Youth Commission partnered with the Center of Health Care Services to offer Teen Mental Health First Aid training.
“It is important to take the Teen Mental Health Survey because it allows teens who are struggling in silence to have a voice and get the support they need,” said Project Worth Teen Ambassador Sophie Perez, representing Council District 5. “This survey allows mental health to be a more talked-about topic in the community and breaks down any stigma.”
Teen Mental Health Survey results for 2022 and 2024 are available on the City of San Antonio website. The city’s announcement about the survey was released on March 12, 2026.
