  1. I think it's important to mention that the risk of a hypertensive crisis is largely overblown. While there are some foods that have a high probability of causing it, most patients can get away without changing their diet much at all. For example, I'm on Nardil, and I eat parmesan and other aged cheeses, chocolate, aged meat, and more, and I've never had a hypertensive crisis. Of course, everyone is different and should start out slowly when trying foods with tyramine while on an MAOI. However, a tyramine-induced hypertensive reaction is not nearly as common as we once thought. MAOIs are incredibly useful for treatment-resistant depression and severe anxiety, and I wouldn't want to scare practitioners away from incorporating them into their toolkits or patients from being willing to take them.

  2. I am a Nurse practitioner in Reno. I have a client that is out of Nardil and has no possibility of getting an in this state. Do you know of a pharmacy that has a supply of the medication? I" am not sure how yo. would contact me. This is urgent, My patient has been out of medications for a week

  3. There's many other side effects like myoclonus, urinary hesitancy, anorgasmia. We need more psych NPs who are aware of all these side effects and how to treat them. The tyramine diet is so simple. It's these other issues that are complicated and that I as a Nardil patient need help with.

  4. I may have missed it. But i didn’t recall info about indications for MAOIs? I want to assume for depression or anxiety but I thought I’d ask!

  5. MAOIs (why do these still exist at all?) highly tested on: phenlazine, tranycypromine, isocarbazid, selgeline. Work by inhibibiting monoamise enzyme causing an increase in neurotransmitter norepi, dopamine and serotonin. SE orthostatic hypotension, anxiety agitation, risk for hypertensive crisis ***** please take it seriously Phenelzine Tranycypromine Isocarbazid Selegine Please Take It Seriously pneumonic. They can cause hypotensive crisis. Tyramines in food can cause the hypertensive crisis, finer foods like guacamole/avocado, pepperoni, aged cheese, wines, bananas, chocolate. Rhyme wi th phenelzine you can't enjoy lifes finer things because they contain tyramines. Avoid those foods and Maois interact with OTC cold meds and can lead to hypertensive crisis. 2 week washout between discontinuing the MAOI and starting the new med

  6. You forgot to mention that MAOIs are used when nothing else works.

Forbidden foods are best described as "aged" staples.
