BOONE, Iowa—Community and Family Resources (CFR) is pleased to announce that beginning on March 31, 2026, our Boone Outpatient office will move to 900 W. Mamie Eisenhower Ave., within the Boone County Social Services/Veterans’ Affairs building. Our office will be closed on March 30, 2026, to complete the move.

Previously located at 1332 S. Marshall St., the same CFR professionals will continue to provide an array of outpatient behavioral health treatment services including substance use counseling, mental health therapy, psychiatry, and gambling counseling.

Our contact information and days/hours of operation will remain the same. Individuals interested in pursuing services at our Boone location may call 515-433-0369 to make an appointment. All appointments scheduled in Boone on or after March 31, 2026, will take place at the new location at 900 W. Mamie Eisenhower Ave.

CFR accepts Medicaid and most health insurance plans, and a sliding fee scale is available for those who qualify. Our programs are accredited by CARF as well as the Iowa Department of Health and Human Services (IHHS).
