hey this is Psychiatry hey this is Ben with medicine I’ve got a console for you okay what’s the story basically a young guy with alcoholic cirrhosis who’s been here for a few days he’s exhibiting some Strange Behaviors and hallucinations okay we’re worried that this is all from an underlying untreated psychiatric illness interesting he’s had multiple admissions for withdrawal in the past but this seems different got it we’re wondering if an underlying psychiatric issue could explain his alcohol issues as well okay let me review the case and get back to you great thanks hello hey this is Psychiatry I’ve got some recommendations oh already wow that was fast yeah so anyway we think this is alcohol withdrawal what it can cause hallucinations and Strange Behaviors sure but his last drink was five days ago this could also be related to the liver this guy has cirrhosis right that can cause unusual behaviors yeah but we’re treating him with medicines for that and it’s been under control it looks like his AST was 65. yeah but that’s not treat the underlying liver problems and the alcohol withdrawal no what about the good luck

Fuck psychiatrists. It’s good to know that real doctors think of them is being worthless too.
At least in this country, if you have a psych problem, you are on your fucking own, because if you go to the psychiatrist/psychologist/therapist, you’re just gonna end up even more fucked than you already are.

This has been the minority of experiences…I’ve seen psychiatrists save people from misdiagnosed health and neurological issues…psychiatry is asked to treat the mind and can’t rely on X-rays and lab results…the are treating a black box that can’t be opened and they rely on whatever is coming out of the black box and what is surrounding the box…they can’t fix a complex situation on their own…
It's the other way around at my hospital. Psychiatry treats withdrawal here.
As a crisis counselor, the number of people I speak to that have given up on mental health care from professionals for this reason is heartbreaking. If people tell me the 30 min convo we have is the most heard they have felt seeking treatment for over 7 years something serious is wrong. It's the most painful when they can get better but I am not equipped to provide the support they need and they have been so scarred by the system that they become hopeless.

Yeah some of them strangely seem to not want to help, like they didn't literally choose to specialize in psychiatrics…
I've had to threaten to hurt myself and worse to not be kicked out of the emergency psych ward, been told that I'm too high functional to need psychiatric help despite literally trying to end my own life and having a history of chronic depression, self harm, anxiety, panic attacks, social anxiety, suicide ideation, dangerous impulses and obvious signs of PTSD, making work and socializing extremely difficult becauseof the social and relational trauma on top of my chronic illnesses, chronic pain and extreme fatigue.

Yep. My last psychiatrist never remembered my name and would spend precisely 5 minutes with me for a session. Even my insurance allowed 15 minutes. Useless.
Had to stop taking anxiety meds (I have anxiety attacks every single day) and depression meds (suicidal) because my psych wanted me to come and see her every single time I wanted my meds refilled. As well as four times a month. So 40 dollars for each visit (eventually would run out of copay so it would be more) plus what I’d have to owe for the meds. So now I’m just rawdogging it : )

Actually alot if mental health issues ARE caused my physical issues. For example narrow spinal cord which is pretty common, can mess up the way the cerebral spinal fluid circulates, causing physical symptoms (which can be so bad it creates emotiona in mental health imbalance) or it can cause mental health symptoms like experiencing hallucinations, irritability, and an over all stress response both mentally and physically.

In fairness, it’s highly difficult to diagnose anything mental health related if there is substance use and medical issues going on. The creator here does a good job of explaining that those factors can be ruled out based on the treatment the patient has received but the limited time frame is still a bit sketchy. All that said, you can get a ton of flavored info from talking directly to the patient.
From 18-26 I was diagnosed with every psychiatric diagnosis from bipolar, BPD, ADHD to anxiety related panic attacks. Turns out, 8 years later and millions in insurance claims that I have Epilepsy. 8 years of benzo's, lithium, anti depressants, anti-psychotics and self medicated substance abuse later, and now I just take one pill (keppra), and it treats all my "psychiatric" symptoms. Psychiatry is a fucking joke. Laughable. They should all pay for their crimes against humanity.
General med MDs: If you become amnestic of basic human physiology, please expect to be summarily dismissed when you call the neurologist for "altered mental status" in a patient with BUN 102, or ammonia of 125 or, yes, alcoholic kept inpatient and dry for 5 days. Please please we beg you, relearn basic sciences and PGY1
MH professional here, and yeah this is 10000% accurate but also I'm not sure what Medicine wants psych to do. 5 days is nothing for alcohol dependence and withdrawal effects can take weeks-months to subside. Even there IS an underlying psych illness, do we then give them psych meds that will mess up their liver even more? Unless this person has significant history of psychosis, neuro should be consulted. Anyway no criticism here, I love the content!!
