Sheridan Memorial Hospital (SMH) Director of Care Continuum and Patient Experience Kelly Lieb and Primary Care physician and Medical Director of Sheridan Green House Dr. Kristopher Schamber made an appearance on Sheridan Media’s Public Pulse highlighting the SMH Transitional Care Unit and its recent relocation to the Sheridan Green House campus.
Dr. Schamber said transitional care is a phase of SMH’s continuum of care for patients who are ready to leave the hospital but need additional rehabilitation before heading home.

The service provides short-term recovery support for patients following illness, injury or surgery.
Lieb said relocating the program to Sheridan Green House offers a more home-like setting while maintaining skilled care services and equipment designed to promote recovery, rehabilitation, independence and successful transitions home.
